Welcome to La Salle Elementary School
Below are documents and items needed to complete the enrollment process. All of us at La Salle Elementary work hard to make our students confident, healthy, and ready to learn. (The learning is up to them!) We hope you will join us at La Salle Avenue Elementary School!
Documents needed to complete enrollment:
♦ Birth Certificate of enrolling child(ren)
♦ Utility Bill (electric, gas, water, internet) or official government mail verifying your address.
♦ Driver’s License or ID card
♦ Completed Health Card
♦ Immunization Record
♦ A report card/transcript from your current school
♦ G.A.T.E., IEP and/or 504 Plan if applicable
